You will be asked to complete a series of questionnaires, which give us different information about your symptoms before and after the procedure.

The questionnaires will ask about:

• your general health and quality of life
• any prostate or urinary symptoms and how these may affect sexual function
• how acceptable you found the procedure
• if you had any specific reasons for choosing the treatments you were willing to receive
• any catheters you have used
• any surgical procedures you have received for your urinary symptoms
• your use of health care services and time away from work caused by illness
• your travel time, costs and time away from usual activities resulting from attending your healthcare visits

You will be asked to complete these questionnaires at your initial screening visit, (with a possible repeat before your procedure) and at 6 weeks, 6 months, 1, 2 and 3 years after the procedure. The questionnaires are short and take just a few minutes each to complete and in total should take between 30 and 50 minutes.

When you consent to the study you will be asked to confirm how you would like to receive and complete these questionnaires. You will be asked whether you would be happy for your anonymised comments to be used as direct quotations in publications. You can either choose to receive the questionnaires via an email invitation with a link, or in the post with a stamped pre-addressed envelope to return them in. You may receive a couple of reminders if you forget to complete them.
