STICH II

STICH

International multi-centre trial is to evaluate the role of early surgery in superficial supratentorial lobar haematomas without intraventricular haemorrhage.

STICH II is a multi-centre randomised parallel group trial comparing early craniotomy to evacuate the haematoma with initial conservative treatment, following spontaneous superficial intracerebral haemorrhage.

Brief inclusion / exclusion criteria

Inclusion:

  • Spontaneous lobar ICH on CT Scan
  • Patient randomised within 48 hours of ictus
  • Surgeon is in equipoise
  • Best MOTOR score on GCS of 5 or 6 and best EYE score on the GCS of 2 or more
  • Volume of haematoma between 10 and 100ml [using Broderick Method (axbxc)/2]

  • Exclusion:

  • Aneurysm, tumour, trauma, angiographically proven AVM
  • Intraventricular haemorrhage
  • Hydrocephalus
  • Brain stem / cerebellar haemorrhage
  • Severe pre-existing physical or mental disability or severe co-morbidity
  • Surgery cannot be performed within 12 hours
  • Unreversed clotting or coagulation problems
  • Staff Involved
